Informed Consent for Treatment

1. Treatment Information

Thriving Center of Psychology offers psychotherapy and medication management services. Therapy is a collaborative process where mental health concerns are assessed and treated using evidence-based techniques.

Benefits and Risks

Active participation is key to achieving your treatment goals. While your provider will guide the process, no guarantees can be made regarding outcomes.

Professional Relationship

Your relationship with your provider will remain professional, and mutual respect is expected throughout the therapeutic process.


2. Telehealth Consent

Telehealth services allow you to receive care remotely via secure video or audio platforms.

Providers will assess whether telehealth is appropriate for your care and may recommend in-person sessions if needed.

3. Medication Management

If deemed appropriate, your provider may recommend medication as part of your treatment plan. Medications will only be prescribed by providers licensed in your state.

Your Responsibilities

Your provider will explain potential benefits, risks, and alternatives to medication use.


4. Confidentiality

Your sessions and treatment records are confidential, with the following exceptions:

Providers may consult with other professionals to enhance your care, but identifying information will be shared only when necessary and within legal guidelines.


5. Records and Documentation

Your treatment records will be securely stored for a minimum of ten years. If you require access to your records, please contact Thriving Center of Psychology.


6. Consent for Minors

For clients under the age of 18, parents or legal guardians must provide consent for treatment. While parents may receive general updates, maintaining confidentiality with the minor client is essential to the therapeutic process.


7. Pre-Licensed Clinicians

Some services may be provided by pre-licensed clinicians under the supervision of licensed providers. These clinicians adhere to the same ethical and legal standards. Your provider will inform you if your clinician is pre-licensed and share the name of their supervisor.


8. Risk Disclosure
